When you use a provider’s contact form, DWI Help Center collects your name, email address, optional phone number, selected provider, submission time, and referral source.

Who receives it

Your request is sent through Web3Forms to The Legal Network, Inc., which operates DWI Help Center. The Legal Network forwards it to the provider you selected, manually or through email forwarding rules. Web3Forms processes the contact details and notification email. Sites provides hosting and storage.

How we use it

The provider uses your contact details to respond. DWI Help Center uses inquiry records to operate the service, track delivery, and measure marketing results. Submitting a request does not authorize distribution to other providers or subscription to marketing messages.

Keep your request limited

Do not submit medical records, substance-use assessment results, payment-card details, or confidential case information. Discuss those matters directly with the provider through its appropriate intake process.

Questions and requests

For questions about a request or to request deletion of information held by DWI Help Center, reply to a DWI Help Center notification email or contact the selected provider and ask it to pass your request to The Legal Network, Inc. The provider maintains its own records under its own policies.
